This is now an online form and only needs to be completed once for the 2016-2017 School Year. So if you have filled it out for one trip you are good to go on all others!

About Consent Forms
MPUMC Parental & Medical Consent forms are required for all youth participating in activities off-site. A new form is required for each trip and the form will be emailed to you along with the packing list and covenant of conduct about a week prior to your youth's trip. Your youth will be expected to turn in the Parental & Medical Consent form along with the Covenant of Conduct prior to boarding the vans or charter bus. This form is not used for mission trips or pilgrimages. This form is available on the website.
