MMPUMC Parental & Medical Consent forms are required for all youth participating in actiivities off-site. Forms expire August 31 of each year and you will have the opportunity to complete a new one each fall. Both parents's signatures are requested, however if one parent is not available to sign the form, the signature of one parent may be witnessed by another adult or a notary. Please note that this form is not used for mission trips or pilgrimages.

Consent forms for mission trips and pilgrimages are trip specific and will be mailed to team members about 8 weeks prior to their trip. The signature of BOTH parents is required and these forms DO REQUIRE a notary. These forms will not be avaiable on the website.
